Application Form Temporary Resident

If you live within the Practice boundary, you can apply to register with us.   If you are staying within the area for under 3 months, a Temporary Resident Application Form should be downloaded and completed.  

You can register in-person at the Practice during normal opening hours.  Alternatively, you can download and hand-in the registration form from our website.  Medical treatment is available from the date of registration.

You can only register with a GP Practice if you are staying in the area for more than 3 months.  If you are not, you may still be treated, but as a Temporary Resident.
